Instructions for Using HowWasYourCare.com

To complete a survey:

1. On the Physician Search page, search for the physician you’d like to rate by entering the physician’s location (ZIP code or state and city), and name or specialty type.
2. Click "Yes" to consent to the User Agreement, then click the “Search” button.
3. A list of physicians matching your search criteria will be displayed.
4. Click on the name of the physician you’d like to rate.
5. Click on the “Complete a Survey” button, under "Patient Experience Survey Results."
6. Answer the survey questions and click “Submit.”

To see survey results:

1. On the Physician Search page, search for a physician by entering their location (ZIP code or state and city) and name or specialty type.
2. Click the “Search” button.
3. A list of physicians matching your search criteria will be displayed.
4. Click on the name of the physician whose survey results you’d like to see.
